OUTPATIENT REHABILITATION

Our Therapists

Our physical, occupational and speech therapists are graduates of accredited therapy programs and are state licensed.  Some hold advanced Master’s and Doctorate degrees in physical, occupational and speech therapy.  In addition, several have advanced certifications, including lymphedema and manual therapy.

Our therapists have extensive experience in treating the conditions which affect active adults.  These include:

  • Musculoskeletal disorders such as arthritis, joint replacement, and post-fracture care
  • Neurological issues such as post-stroke, Parkinson’s and neuropathies
  • Cardiac, pulmonary, cancer care and fall prevention
  • Lymphedema

Our Approach Begins with You

Our approach is simple:

  • Your care begins with a complete assessment of your condition or injury in a private setting
  • Your plan of care is developed with you and personalized to meet your goals
  • Your progress is set at your pace.  We will work with you to help you achieve your goals as your body improves
  • Your progress and return to an active lifestyle is our highest priority
  • Your schedule is important to us.  Treatment times are scheduled at your convenience

How do I become a client?

A physician’s prescription for rehabilitation is required in order to initiate therapy.  The prescription needs to be signed and dated by the referring physician, and include a diagnosis and the discipline (i.e., physical therapy, occupational therapy, speech therapy).

A member of our scheduling team will call you to confirm receipt of your information and arrange your initial appointment.

What should I bring to my first visit?

Plan on arriving 15 minutes early in order to complete any paperwork that may be required.  Please bring your insurance card, a photo ID, a list of current medications, and any other documents the scheduling representative may have requested when you made your appointment.

What should I wear?

Please wear comfortable, loose clothing that will allow you to easily expose the area being treated and to exercise comfortably.  Sneakers or rubber soled shoes are also recommended.

How long is each treatment session?

For physical, occupational and speech therapy, initial evaluations are scheduled for 60 minutes, with subsequent visits lasting approximately 45 minutes.  Most of your treatment time is spent one-on-one with your therapist.

Will I see the same therapist each visit?

We understand that continuity of care is important to patient outcomes.  We will do our best to assign responsibility for your care to a primary therapist.

How long will I need therapy?

After your evaluation is complete, you and your therapist will establish your individualized plan of care.  The average course of treatment varies by diagnosis, but generally can last from 2 weeks to 6 months, based upon your individual condition.  The therapy plan may be adjusted according to your progress and the need for ongoing skilled care.

You may be instructed on a home exercise program which you will be advised to perform throughout the course of your therapy.  This personal exercise routine will better prepare you for an individual discharge plan.  Your therapist will modify and update your program as needed and as you make progress.  Ongoing communication with your therapist and referring physician is encouraged to prepare you for your discharge from outpatient therapy.
